Medical Records
Medical Records request must be in writing and need to be on the AUTHORIZATION TO USE OR DISCLOSE HEALTH INFORMATION PURSUANT TO HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT OF 1996 (HIPAA) form (link located below). We cannot accept requests for medical records through email or the Internet.
Student Health will respond to all routine requests for medical information within two (2) working days after receiving a completed AUTHORIZATION TO USE OR DISCLOSE HEALTH INFORMATION PURSUANT TO HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT of 1996 (HIPAA) form. If, in Student Health's judgement, a request for the release relates to a medical emergency or urgent situation, we will respond as soon as possible.
Once you become 18 years old, as a student, you have the right to privacy and confidentiality regarding your medical care.
Occasionally Student Health receives a signed statement from a student stating that he wishes his medical information to be released to his parents. Student Health requires the student(patient) to complete and submit an AUTHORIZATION TO USE OR DISCLOSE HEALTH INFORMATION PURSUANT TO HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT OF 1996 (HIPAA) form. The form must be notarized. If you complete the form and return it to us, we will be happy to respond to your request, including request to release information to your parent(s).
Medical records are saved for ten (10) years after the date of your last visit to Student Health. If you have never been seen by a Student Health provider while enrolled at ·¬ÇÑÖ±²¥app, your immunization records are kept for two (2) years from the date you enrolled after your departure from this university.
There are times when a student would just like a copy of their immunizations. If you are a current student at ·¬ÇÑÖ±²¥app, you can obtain a copy of your immunizations by using our online appointment system at .
